OUR OFFICE IS ONLY OPEN TO STAFF AND THOSE WHO HAVE MADE PRIOR ARRANGEMENTS FOR IN-OFFICE APPOINTMENTS.
Please read below for information on how we are proceeding with specific services.
CURRENT PATIENTS
All group and one-on-one sessions will be conducted using Zoom's HIPAA-compliant teleconference system. All patients with an email address will receive an invite to their online session, which will occur at its regularly scheduled time. If you do not have an email or access to a computer, smart phone, or the internet, please call our office for the phone number and meeting code to join via phone.
PAYMENTS
Payments can be made over the phone with a debit or credit card during limited business hours, or online at any time. We are not accepting cash payments at this time.
Beginning 03/24/2020, all intakes will be done over the phone. Due to current limitations on obtaining documents from other agencies, there may be difficulties scheduling intakes for those who were assessed elsewhere. Please call for more information on how to obtain docs and get your appointment scheduled.
DRUG/ALCOHOL ASSESSMENTS
Assessments will be done over the phone after the following steps have been taken:
1. Call during our limited business hours to schedule and pre-pay for your appointment.
2. Take pictures of your court documents and ID and email them to sue.axon@affinitycounseling.net PRIOR to your appointment.
3. Check your email for the assessment handout and keep it handy for your appointment.
4.Be available to accept phone calls at your scheduled appointment time. The counselor will call once at the scheduled time and again five minutes later if they do not reach you on the first attempt. Keep in mind the number will be anonymous.
You will not be called if payment has not been received.
DV & ANGER ASSESSMENTS
We are now able to provide Domestic Violence and Anger Management assessments in-person for those who are comfortable doing so. Masks are required and social-distancing protocols will be followed. Unfortunately, we are unable to perform these assessments over the phone.
ADIS
All education courses are cancelled until further notice. If you have already registered, you may call the front desk to request a refund, or ask that it be credited to the next available session. If you would like to be placed on a wait list, call during our limited phone hours and let us know.
